The legal next of kin must sign all NYC Funeral & Cremation Service paperwork. The legal next of kin must certify that he/she is not aware of any other closer legal next of kin to the Decedent. If the legal next of kin is knowingly misrepresenting himself/herself as the closest legal next of kin to the decedent, the legal next of kin will be responsible for any legal costs that arise from this misrepresentation. NYC Funeral & Cremation Inc. Service has the legal right to sue the legal next of kin that signs paperwork for all the damages and costs due to misrepresentation.
The name at the top of the death certificate is put in by the doctor, or medical examiner and this cannot be changed by the funeral service. If the Decedent’s name that you provided on the vital statistics form is different then what the doctor put on top of the death certificate, the name that you gave us is put in Box 16 (Aliases or AKA), on death certificates from NYC. On NY state death certificates, AKA is in box 1. If you want the decedent’s name to be changed then please contact the doctor or medical examiner because they are the only ones that can change the name. The Decedent will not be removed until the name has been fixed so this will delay removal.
If Decedent was Medicaid eligible in NYC, we will assist you in getting up to $1700.00 reimbursement on a cremation. Due to the impact of COVID-19, the HRA allowance has been increased. Full payment must be made by the family to the funeral home. The $1700 reimbursement from HRA is paid directly to the family if Decedent was on Medicaid in NYC.
PLEASE NOTE: The HRA reimbursement is paid directly to the family member that paid for the cremation. We do not accept payment directly from HRA. We do not participate in this program. We will provide all the forms you need for the HRA reimbursement when we mail the death certificate. Extra charges if Decedent is not released to us due to discrepancy in next
of kin that was listed on the vital statistics. You will be charged an extra removal fee.
WE DO NOT RE-ORDER DEATH CERTIFICATES. The family goes directly online to www.nyc.gov to order death certificates if the death occurred in NYC (all five boroughs). If an informant provided incorrect information on the vital statistics, there is a service charge for us to amend the death certificate for both NYC and NY state.
Please do not schedule a memorial service until you have the cremains of your loved one in your physical possession. NYC Funeral & Cremation Service Inc. is not responsible for any delay in the delivery of cremains to you.
